St Bartholomew's Medical Centre

We register new patients who have moved to live within our practice area which generally speaking is within the OX3 and OX4 postcodes. Any patient staying less than three months may be able to register as a Temporary Patient.

All new patients are asked to complete a medical questionnaire and registration form. We need these details on your computer record before you see a doctor. These forms are available below, please print off the relevant form, complete them, sign and bring to the surgery with you.

Alternatively, you can ask for a form from our reception team.

You cannot register for someone else unless they are with you and this includes children.

For purposes of identification you will need to bring your passport and proof of address. Proof of address will need to be a utility bill (gas, electricity or water) or bank statement, credit card bill etc. dated within the last six months.

If you have any queries or are unsure what to bring please talk to our reception staff who will be happy to assist you.

Change of Address or Telephone Number

It is important to keep your address and contact details up to date to avoid being taken off our list.

Please notify us immediately of any change of address or telephone number. This prevents problems when sending you letters, making hospital appointments or home visiting.


New regulations also allow NHS England to check your registration, and if they do not receive a reply they will remove you from our list as ‘Gone Away’.

If you are a student at Brookes, please note that you MUST notify the Medical Centre of your new address, as the University does not pass on that information.
