Change of Details
If
your details have changed in any way, please advise the Medical
Centre. You can do this either at St. Bartholomew's Medical Centre
or in the case of Oxford Brookes University students, you can
also do this at the Oxford Brookes Medical Centre.

is important for all patients to advise us of their most upto
date details as often the practice may need to contact patients
either by post or by phone. Wrong addresses leads to missed appointments,
lost letters and inefficient communication.
